Your affidavit (exemption request form) will be mailed to the USPS address you provided.

Note that it may take up to three weeks for affidavits to be mailed to you. Please do not resubmit your request, as repeated affidavit requests slow the fulfillment process.

After receiving your affidavit, it will need to be notarized and submitted to your child's or student's school or day care.

Do not send affidavits to Texas DSHS.
